A private, concierge-level senior care management and oversight service. We are the professional stand-in for family members who cannot be present – whether because they live out of state, work full-time or simple need a trained expert to be their eyes and ears. We attend appointments, assess living environments, coordinate care, advocate at facilities, and ensure seniors are safe, well-cared for, and heard.

Core Service – Ongoing care management & oversight:

This is the heart of what Premier Path does. Families hire Premier Path on a monthly retainer to be their professional, local representative for a senior living in Fort Wayne – whether that senior is at home, in assisted living, in memory care, or in any other setting. The retainer covers a defined set of visits and services each month. Families always know exactly what they’re getting and what it costs. 

Who this is for:

Adult children who live out of state or out of the area. Families who work full-time and cannot attend appointments or check in regularly. Families who want a clinical professional – not just a friendly visitor – monitoring their loves one’s wellbeing and peaking up when something is wrong. 

Services: 

Every relationship starts with an assessment. From there, you choose the level of ongoing support that fits your family.

Step One: Comprehensive Care Assessment – $450

This is where every client starts, no exceptions — it’s what makes everything after it accurate.

  •     In-person visit with your loved one, wherever they are — home, facility, or hospital
  •     Full evaluation of mobility, memory and cognition, daily living skills, and fall risk
  •     Walk-through of the home or living space for safety and equipment needs
  •     Review of current medications and care plan
  •     Written report with clear, prioritized recommendations, delivered within 5 business days
  •     30-minute call to walk you through the findings together

Fee is $450, due at booking. If you enroll in a monthly plan within 30 days, this fee is credited toward your first month.

All plans are pre-paid monthly. Travel within 15 miles of Southwest Fort Wayne is included at no extra cost. Visits beyond 15 miles include a $35.00 travel fee, added to your monthly invoice. Any support needed beyond the included visits is billed at $175/hour. A 30-day written notice is required to cancel services.
