Once your initial Comprehensive Care Assessment is complete, choose the tier of ongoing care management and advocate support that fits your family’s needs.
Essential – $900/month
Best for stable seniors who just need consistent professional eyes on them.
- 2 in-person visits a month, up to 90 minutes each.
- Written summary after each visit.
- 1 family update call per month.
- Up to 2 coordination calls a month with facility staff or providers.
- Responses within 1 business days.
- Appointment attendance and crisis coordination available as add-ons.
Premier – $1,800/month
Best for families who want Premier Path showing up consistently, attending appointments, and handling communication so you don’t have to.
- 4 in-person visits a month, up to 90 minutes each.
- Written summary after every visit.
- Attendance at 2 medical or therapy appointments.
- Unlimited coordination calls with facility staff, physicians, and providers.
- 2 family calls per month.
- Responses within 1 business days.
- Crisis Coordination included.
Concierge – $3,800/month
Best for families who want Premier Path to function as their full local representative.
- Up to 8 in-person visits a month.
- Unlimited medical and therapy appointment attendance.
- Full care coordination – single point of contact for every provider.
- Nutrition & grocery oversight.
- Weekly family updates.
- Monthly comprehensive written report.
- Response time within 2 business hours, Monday – Friday.
- Crisis Coordination included.
All plans are pre-paid monthly. Travel within 15 miles of Southwest Fort Wayne is included at or extra cost. Visits beyond 15 miles include a $35.00 travel fee, added to your monthly invoice. Any support needed beyond the included visits/time is billed at $175/hour. A 30-day written notice is required to cancel.
A La Carte Options
For families who aren’t ready for a monthly plan, or just need something specific.
- Home Safety Assessment (standalone) – $500: Full OT-informed safety walkthrough with written report, no ongoing commitment required.
- Hourly Advisory Consulting – $200/hour (1-hour minimum): One-time guidance, credited toward your first month if you enroll within 30 days.
- Companion Visits – $85/hour: Social visits, grocery runs, appointment accompaniment. Separate companion agreement required.
Senior Living Placement
If a move to assisted living, memory care, independent living, or skilled nursing becomes the right next step, Premier Path manages the entire process so you don’t have to navigate it alone.
- Free for Active Clients: Included at no additional cost for families currently enrolled in an Essential, Premier, or Concierge monthly plan.
- $1,200 Flat Fee for Non-Enrolled Families: Complete placement search, matched community recommendations, tour scheduling, side-by-side comparisons, move-in coordination, and a follow-up visit within a week of move-in – with no ongoing monthly commitment.
