Private, Concierge-Level Senior Care Management & Oversight
A private, concierge-level senior care management and oversight service. We are the professional stand-in for family members who cannot be present – whether because they live out of state, work full-time or simple need a trained expert to be their eyes and ears. We attend appointments, assess living environments, coordinate care, advocate at facilities, and ensure seniors are safe, well-cared for, and heard.
| What Others DO | What Premier Path DOES |
|---|---|
| Placement agencies find a facility, collect their check and move on | We stay involved – visiting, monitoring, advocating, and reporting back to family. |
| Home health aides provide hands-on care but no clinical oversight or advocacy. | We assess the full environment – home safety, functional needs, routines, equipment – and coordinate the right way. |
| Families rely on facility staff to self-report problems. | We are physically present and independently verify what’s happening. |
| Out-of-town families get secondhand information and feel helpless. | We are their local professional representative – attending appointments, taking notes, and communicating directly. |
| Generic assessments with no functional lens | OT-informed assessment of what the senior can actually do, what support they need, and what environment serves them best. |
Core Service – Ongoing care management & oversight:
This is the heart of what Premier Path does. Families hire Premier Path on a monthly retainer to be their professional, local representative for a senior living in Fort Wayne – whether that senior is at home, in assisted living, in memory care, or in any other setting. The retainer covers a defined set of visits and services each month. Families always know exactly what they’re getting and what it costs.
Who this is for:
Adult children who live out of state or out of the area. Families who work full-time and cannot attend appointments or check in regularly. Families who want a clinical professional – not just a friendly visitor – monitoring their loves one’s wellbeing and peaking up when something is wrong.
